There was no history of ocular trauma or surgery. On ocular examination elsewhere, she was diagnosed as having intravitreal cysticercosis after observing a cyst in the left eye and was referred for vitrectomy to a tertiary eye care centre. Her vision was 20/20 in both eyes, with no signs of ocular inflammation. Biomicroscopic examination of the left eye showed a mobile, pigmented, circular cyst in the anterior vitreous, but it was not blocking the pupillary axis (figure 1). Intraocular pressure and fundus examination of both eyes was normal. Ultrasonography revealed a free-floating cyst with no scolex in the anterior vitreous. Ultrasound biomicroscopy did not show any other cyst or mass in the iris and ciliary body. Non-contrast head CT of the brain and orbit ruled out intracranial cysticercosis. The headaches were attributed to convergence insufficiency and the patient was advised orthoptic exercises. It is easy for a floating iris cyst to mimic intraocular cysticercosis, as it is a common diagnosis, and also, most surgeons are unaware about iris cysts. Often, neuroimaging is normal and adds to the diagnostic dilemma. Although both appear quite similar, the classic pigmentation on the cyst wall, lack of ocular inflammation and lack of scolex on USG help confirm diagnosis of the floating iris cyst. Shield postulated that epithelial cysts arise between the pigmented epithelial layers of the iris, and often break free and migrate into the vitreous chamber, and these are called dislodged cysts. Lois et al reported that 12% of primary iris pigment epithelium cysts were vitreous cysts. Sugar et al suggest it is important to distinguish retrolental iris cysts (between lens and anterior hyaloid) from anterior vitreous cysts—as retrolental cysts may cause visual disturbance and may be associated with posterior lenticular opacities.

Free-Floating Iris Cyst in a Patient with Recurrent Iritis
PURPOSE We describe an unusual clinical finding of a free-floating iris cyst in a patient with recurrent iritis. METHOD The clinical finding of a free-floating iris cyst was recorded using slit-lamp photography. RESULTS A 39-year-old male with a 5-year history of recurrent right iritis was found to have a small mobile iris cyst within his right anterior chamber, first identified 3 years ago...
متن کاملExtracting a large live freely floating cysticercosis cyst from the anterior chamber of the eye using visco expression technique: A case report.
Ocular involvement by cysticercosis is uncommon and rare in the anterior chamber. It can give rise to iridocyclitis which can be potentially blinding to the patient. The management is usually surgical. We report a case of 18-year-old girl with large cysticercosis cyst in the anterior chamber. The cyst was removed intact by viscoexpression technique from the anterior chamber of the eye and the p...

A 50 year-old male patient was referred to our clinic due to a floating mass in the right eye. The uncorrected visual aquity was 10 / 10 in both eyes. The patient did not have any systemic disorder and trauma history. His ophthalmological examination revealed an unremarkable anterior segment with no signs of inflammation. Indirect opthalmoscopy and posterior segment biomicroscopy performed with ...
متن کاملFree-Floating Iris Pigmented Epithelial Cyst in the Anterior Chamber
An unusual case of a free-floating peripheral pigmented cyst in the anterior chamber is presented. A 30-year-old Caucasian male presented reporting a visual defect on his right eye in prone position over the past year. Slit-lamp examination revealed a small pigmented free-floating peripheral iris cyst at the 6 o'clock position in the anterior chamber. Ultrasound biomicroscopy revealed an unfixe...

SUMMARY Iris cyst occurs post intraocular -surgery or post penetrating trauma, when corneal epithelial cells or conjunctiva} epithcells gains access to the anterior chamber, with cells may continue to grow & covering all available surface and formed iris cyst.
